Ongoing management of hypertension, diabetes, heart disease, COPD, arthritis, and other chronic conditions prevalent in older adults.

Memory and Cognitive Screening

Structured cognitive assessments to screen for early memory loss, dementia, and cognitive decline with specialist referrals as indicated.

Psychiatric Care for Elders

Evaluation and treatment of depression, anxiety, insomnia, and other mental health conditions that commonly affect the elderly population.

Functional Status Evaluation

Assessment of activities of daily living, mobility, and independence to guide appropriate care planning and support coordination.

Care Coordination

Coordination with home health agencies, physical and occupational therapists, skilled nursing facilities, and specialist providers.
